Charges were filed four months after the collision happened on Oct. 8.
Kyle John Bahensky of Ontario faces one count of gross vehicular manslaughter while intoxicated, two counts of driving under the influence causing bodily injury, and one count of hit and run resulting in injury or death, according to court records.
The October crash, involving a pickup and a sedan, happened at Campus Avenue and Arrow Highway in Upland. Joshua Roberson, riding in the sedan, was killed in the collision.
Bahensky, who police believe was driving the pickup, was arrested after authorities tracked him at his home.
He was booked into West Valley Detention Center in Rancho Cucamonga on suspicion of felony hit-and-run, but was released after posting bail, according to San Bernardino County Sheriff's Department booking records.
Bahensky has pleaded guilty or no contest to three traffic violations in the past three years, according to court records. He was ticketed in April for speeding in July 2010 for using a cellphone while driving, and in July 2009 for running a red light, court records show.
